Checklist for Developing a Logic Model

Review the basic framework of the program including its mission, goals, objectives, and strategic plan.
Identify the context in which the program takes place, including any factors that might influence its outcome.
Describe the program with a logic model by identifying the inputs (resources), outputs (activities, participation), and the outcomes (short-term, intermediate, long-term).
Review the logic model with key stakeholders to ensure you have a common understanding of the program's elements as you get ready to plan your evaluation.
