Checklist for Focusing the Evaluation Design

Now could be a good time for Healthcare Georgia Foundation grantees to contact the ERC for assistance with putting together the pieces of your evaluation plan.

	Review the logic model with stakeholders and identify what elements of the program (inputs, outputs, and outcomes) will be most important to evaluate given the program's stage of development and available resources.
	Use Worksheet 3: Identifying and Prioritizing Evaluation Questions to identify what you and your stakeholders want to learn from the evaluation and how that information will be used.
	Use Worksheet 4: Developing Indicators to brainstorm possible indicators for each question from Worksheet 3. Rate the indicators and select your final evaluation questions and indicators.
	Enter the final evaluation questions and indicators in Worksheet 5: Evaluation Plan.
